1. Why this policy exists
State consumer-health privacy laws (such as Washington's My Health My Data Act and Nevada SB 370) treat health-related data collected by a website as its own category, with its own notice. This page is that notice, separate from the general privacy policy.
2. Consumer health data this site collects
The only health-adjacent data this website can receive is the brief visit reason you may type into the appointment form. The site sets no cookies, runs no analytics or advertising trackers, and makes no third-party requests — so no health inference can be drawn from your browsing here, and none is shared, because none is captured.
3. How it is used, and not used
Form submissions are used only to schedule your visit. They are not sold, not shared with advertisers, and not used to build profiles. They go straight to the practice's own scheduling inbox — no form vendor, ad platform, or analytics system sits in between — and are deleted once your visit is booked.
4. Your rights
Where state law applies, you may ask what consumer health data we hold about you, ask us to delete it, and withdraw any consent you gave. Contact privacy@alderfamilymedicine.example and we will acknowledge within two business days and respond within thirty.
5. Medical records are separate
Your medical record is not part of this website. How the practice handles health information as a provider is described in our Notice of Privacy Practices, available at the front desk and on request.
